NOFA for Rural Telemedicine Technology - Stanislaus County California

Good Day:
I have spent close to 4 full days researching multiple web locations (including the BTOP outreach page) seeking some concrete answers on how my rural communities can partner to apply for
stimulus Healthcare IT grant dollars for much needed telemedicine connectivity.
Can you please assist me (direct me) to actual application process locations?
