National Telecommunications and Information Administration Minority Telecommunications Development Program 2000 Survey of Minority Broadcast Owners

The purpose of this questionnaire is to collect information from individuals who owned radio and/or television broadcast stations during the period of July 1, 1999 through June 30, 2000.

The business owner(s) should complete this questionnaire even if the business has been sold, reorganized, or discontinued.

If you have not received a reference number in the mail but would still like to answer the questionnaire, please call the Minority Telecommunications Development Program at (202) 482-8056 between 8:30 a.m. and 5:00 p.m. eastern standard time.

Notwithstanding any other provision of law, you are not required to respond to any collection of information subject to the requirements of the Paperwork Reduction Act unless that collection of information displays a currently valid control number from the Office of Management and Budget. The control number appears at the top of this page.
