Application Form

Applications must be received by
5 p.m. Friday, February 22, 2008.

All applicants must use the APPLICATION FORMS which are available on this web site. Previous editions of the PTFP forms, which expired 10/31/2006, are NOT USABLE and will be returned.

The PTFP application forms are available on this website in two formats:

I.	We recommend completing the application forms listed in sections A and B through the Web.
A.	NTIA's web-based fillable forms.
	After login, the following forms can be completed and saved. No special software is required. The four application forms shown below can be completed and saved on-line with the fillable form. An instruction icon is available at each qestion. The answers for many questions are pre-filled; other questions provide selection boxes for quick click selection. These forms, when finalized, can be printed for duplication, signatures and submission. Applicants for Construction Projects can use the fillable forms to enter equipment requests and related costs; the form will automatically calculate totals and carry dollar figures to all appropriate sections and will complete the SF-424A Budget Form. (Refer to appropriate project guidelines for complete instructions.) Applicants for Planning Projects can use the fillable forms, but must enter the dollar amounts for the proposed project costs and the requested Federal share in the appropriate places. (Refer to planning guidelines for complete instructions.)

Information entered is not saved and should be printed as each form is filled. (Note: Signatures, where required, must be filled in manually, with an original signature.)

When you contact PTFP, please indicate the type of project you are applying for--1) new broadcast station, 2)equipment replacement, 3) nonbroadcast, 4) planning, or 5) digital conversion--so we may send you the correct information.
